Frequently Asked Questions
Please follow these steps to prepare for your appointment:
- New patients should access the patient portal and complete the questionnaires at least seven days prior to your appointment.
- Please provide us with medical records at least five days before your appointment. You can fax them to (239) 221-0469 or upload them to the Patient Portal. Your physician will review your medical history before your appointment. Medical history for one year is sufficient. Please bring your current vitamins, medications and supplements in their original containers if possible. Please also bring any recent labs or X-rays.
- Confirm your appointment within a 24-business-hour minimum notice. If you need to cancel your appointment, please see the cancellation policy section.
- Please arrive at least 15 minutes before your appointment time to complete any necessary paperwork. Our office strives to remain prompt and we do not want your time with your physician to be cut short.
- Please do not wear any scented products or nail polish to your appointment.
Please plan for a total appointment time of approximately three hours (not including travel to and from the office).

Pre-Consultation: 10 minutes

Physician Consultation: 60 – 90 minutes
- During your consultation, your physician will go over your medical history and consult with you your options and recommendations to begin your path to long-term health.
- Your physician will work with you to create an initial treatment plan and discuss your recommended laboratory testing and/or infusions.
- It is recommended that you come prepared with all questions for your physician.
Laboratory Testing and Infusions: 15 – 60 minutes
- Depending on the amount of testing or infusions needed, this process could take anywhere from 15 minutes to one hour.
Follow-up Appointment: 30 – 60 minutes
- After your initial appointment, you will be asked to schedule a one to two-month follow-up appointment.
- During this appointment your test results and progress will be reviewed and recommendations for any additional screenings or consultations may be discussed. If an additional wellness and nutrition consultation is recommended, instructions will be provided for scheduling the virtual appointment and additional costs associated with the wellness and nutrition screening.
However, we will provide a detailed receipt for services performed for you to submit to your insurance carrier.
As of October 1, 2022, an annual medical membership fee applies per patient as well as hourly consultation fees. Concierge membership packages are also available.
For new patient appointments, we require a credit card to be placed on file at the time of scheduling. Seven days prior to the appointment, patients will receive a call to confirm their appointment. If the appointment is not confirmed at least seven days prior to the first visit, or if the initial appointment is canceled within seven days, the patient will be charged 50% of the new patient appointment fee. Once the appointment is confirmed by phone or email, text message and email reminders (if desired) will be sent two days prior to the appointment. For follow-up appointments, payment is due at time of service and cancellations are required no less than 48 business hours.
For new patient appointments, cancellations must be received one full week before the date of your appointment. If you fail to cancel at least seven days prior to your appointment, your 50% deposit will not be refunded.
For a follow-up appointment, cancellations must be received 72 business hours before the date of your appointment or a $150 fee will apply.
Most tests are performed at Naples Center for Functional Medicine. During your medical consultation, your physician determines which tests are needed. Then our nurses review with you testing recommendations, instructions and costs. You do not need to have any bloodwork done in advance of your appointment, nor do you need to fast.
You may choose to receive additional testing if desired. We frequently test for nutrition, stress, genetics, allergies, acids and more. Urine, saliva and stool testing may be done at home.
Keep in mind that you may not require immediate testing to receive effective care.

Southwest Florida International Airport (RSW) is about 30 miles from our office. Rental cars or a shuttle are easily accessible.
Yes. You are required to maintain a primary care physician for all urgent care and annual routine wellness check-ups. You cannot be seen by Naples Center for Functional Medicine without a primary care physician on record.
Our office hours are Monday – Thursday, 8:00 AM-5:00 PM and Friday 8:00 AM-2:00 PM. If you call after hours, our office staff will return your call on the next business day. When leaving a message, please be brief and include the following information:
- Full name, spell last name, and date of birth
- Reason for call
- Best time to be called back
- Phone number
- Email address optional
You can also contact us via the patient portal. Please allow a 72-hour maximum response time for your message and be aware that we will get to your message as soon as possible.
Please be mindful of the physician’s time and limit your questions to your follow-up appointment when possible. Additional messages may result in fees charged per message. For emergencies please call our office directly or 911.
